What Is Deep Vein Thrombosis (DVT)?

Deep Vein Thrombosis (DVT) is a condition where a blood clot (thrombus) forms in one or more of the deep veins, usually in the legs. It can cause pain, swelling, and redness in the affected leg. The most serious complication is Pulmonary Embolism (PE) — when part of the clot breaks off and travels to the lungs, which can be life-threatening.

Types & Grades

Distal DVT

Clot confined to calf veins below the knee. Lower risk of pulmonary embolism. Managed with anticoagulation therapy and compression stockings for 3 months.

Proximal DVT

Clot in the popliteal, femoral, or iliac veins. Higher risk of PE. Requires anticoagulation for 3-6 months, with possible catheter-directed intervention.

Iliofemoral / Massive DVT

Extensive clot in the main leg or pelvic veins. Risk of limb-threatening complications. Requires urgent catheter-directed thrombolysis or mechanical thrombectomy.

Anticoagulation Therapy

First-line and cornerstone treatment. Blood-thinning medications (heparin, rivaroxaban, apixaban, warfarin) prevent the clot from growing and reduce the risk of pulmonary embolism. Duration is typically 3-12 months depending on the cause and risk factors.

Catheter-Directed Thrombolysis (CDT)

A thin catheter is advanced directly into the clot under imaging guidance, delivering clot-dissolving medication (tPA) precisely at the clot site. Most effective for extensive proximal DVT within 14 days of onset.

Mechanical Thrombectomy

A catheter device physically breaks up or aspirates the clot. Used for massive DVT threatening limb viability or when thrombolysis is contraindicated. Performed by expert interventional vascular specialists.

IVC Filter Placement

An inferior vena cava filter is inserted to trap clots before they can reach the lungs. Recommended for patients with recurrent PE or those who cannot safely take anticoagulants.
