What Is Hydrocele?

A hydrocele is the accumulation of fluid around the testicle inside the scrotum, causing painless scrotal swelling. In newborns, it is very common and usually resolves within the first year of life. In adults and older boys, a hydrocele can develop secondary to injury, infection, or inflammation of the testicle, and requires surgical treatment.

SurgiKure performs hydrocelectomy — surgical drainage and complete removal of the hydrocele sac — as a day-care procedure under general or spinal anaesthesia. A small scrotal incision is made, the fluid is drained, and the hydrocele sac is removed or everted to prevent recurrence. Patients go home the same day.

Common Symptoms

Painless swelling of one or both testiclesFeeling of heaviness or fullness in the scrotumSwelling that may vary in size throughout the dayTranslucent swelling when illuminated (transillumination)Mild discomfort with large hydrocelesScrotal skin appears normal and non-tender

Types & Grades

Congenital / Communicating Hydrocele

Connected to the abdominal cavity. Fluid flows in and out causing size variation. Common in infants; usually resolves by age 1-2 without surgery.

Non-Communicating Hydrocele

Fluid trapped around the testicle with no abdominal connection. Common in adults. Requires hydrocelectomy when causing discomfort or enlarging.

Reactive Hydrocele

Develops secondary to epididymo-orchitis, trauma, or testicular tumour. Requires treatment of the underlying cause and hydrocelectomy.

Treatment Options

1

Hydrocelectomy (Surgery)

Gold standard. A small incision in the scrotum allows the fluid to be drained and the sac to be removed or everted. Day-care procedure, excellent results, very low recurrence.

2

Aspiration and Sclerotherapy

Fluid is withdrawn by needle and a sclerosant agent injected. Less invasive but much higher recurrence rate (30-50%) than surgery. Used for patients not fit for surgery.

3

Watchful Waiting

For small, asymptomatic hydroceles in adults, monitoring without treatment is appropriate. Surgery is recommended if it enlarges or causes discomfort.
